The Change of Control Notice is a document submitted to the New Hampshire Director of Charitable Trusts pursuant to New Hampshire RSA 7:19-b. It includes a summary of the proposed affiliation between CMC Healthcare System (CMCHS) and Dartmouth-Hitchcock Health (DHH), material terms of the affiliation, a copy of the Affiliation Agreement, and minutes, financial statements, and other documents from the governing bodies of the parties. It was supplemented and restated on January 21, 2010.
